BD PHP Bugs and minor features V2

I have an online extranet site within brokerdrive.co.uk. There is a bug that needs fixing - where the user can add or delete rows within a form is not performing correctly. In addition there are several very minor feature changes mainly text and registration form questions - not functional type upgrades. I need this done very urgently. This requires a very experienced, capable and reliable freelancer that has first class knowledge of PHP. In summary 1) Fix bug for one form where user can add/delete rows. See attached comm form bug doc. There are about 45 classes where the user can breakdown between multiple rows. Plus make totals in first row bold & not accessible to users. Users cannot change but I do not want the user to tab into these fields. There is also one field that is not working. 2) Change text for some words in above form 3) Add a Salutation question in registration form / remove 1 X Question. 4) Change 1 X reg form Q 5) Put a drop down list into alpha order 6) Fix a couple of spelling mistakes
