code a page with validation

This project is very easy which includes 1. Allow customer to fill in a form with multiple rows, a row contains a set of predefined fields 2. multiple rows can be dynamically added to the form 3. data validation need to be done - using sth like regex 4. information collected would be emailed to a particular email address 5 for hanlding the UI above, ajax and jquery would be used - so the "add row" operation would be using jquery ajax
