Build Word Documents from fragments stored in SQL

We are auditing a large number of organisations and producing detailed reports for them - these reports are currently manually assembled by cutting and pasting the relevant sections from a master word document with legally checked text.
We wish to automate the process - the User should be able to open MS Word 2003, click a button on the toolbar, and be presented with a series of questions. The answers to these questions will determine which passage of text and image is used for each section of the document.
The questions, possible answers and appropriate sections of text, and the path to the required images (small header images, and some example diagrams associated with each section of text) are stored in a SQL database (table structure is available on request, and is flexible) Suitable Stored Procedures can be added on request.
We require the project to be developed in either VBA, or in VB6 using COM and the Word Automation Object. Our other systems use ADO for database connection, so this is preferred.
The code should be well documented, and not use any third party controls or libraries as we expect to modify it further internally.

UPDATE 2: A comment that keeps coming up - This is the first project we are doing here at RentaCoder. If the outcome of this project is good, we will be adding additional features (multiple documents, an editor for the questions/responses etc).
