Customer Application A business Tracking program! Input Fields required: Customer name, address, phone # city state zip blah blah... Dropdown with: Services INsurance Divorce Bankruptcy Defensive Driving Immigration Documentation Each item on the drop down will display the basic fields, Name address etc, todays date, But when one of the items on the dropdown is chosen, then cutomized fields for each dropdown item are required for example: In Insurance, we would need fields such as Policy #, Effective Date, Cancellation Date, comments, Insurance company name, downpayment, quoted premium, actual premium ....Commission Received, Commission Earned Comission returned, All the commission fields would have a date associated with them. And in Bankruptcy: Additional Fields would be: Downpayment, Date Petition Filed, Court Date: Court Costs, preparation costs. Need to be web based, and have certain requirement will include: 1: Ability to track customer by job type 2; Calculate total revenue in total and by specific item (such as how much income in Insurance, or Bankruptcy, etc.) 3: Search ability buy fields... 4: Totally intergratable to a web based invironment. 5: Allowing customers to view customized fields ( Seperate then what Admin has, for instance we would not want a customer to see the Commission Field in the Insurance Area...) ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. ## Platform Unix or NT is fine.
