Add Testimonial Feature

Hey expert, I need an add your testimonial, name/contact info and before and after pictures feature on my site so my members can submit their testimonials, name/contact info and before and after pictures. I need this feature integrated so there is some way I can go over them and I will be adding them to my site by my choice. I don't want anything extravagant or over the top just to get the job done.
