Convert PDF Brochure to Web Site

I have a PDF brochure which I need to convert to a simple, clean web site within about 7 days of the bid being accepted. Please find the PDF attached. I have hosting already arranged so all I need are the files and images to post. Below is a list of the major points to consider for this project:
1) All work to be completed and delivered with 7 days of the bid acceptance.
2) Work to be done using ASP and/or HTML files as my hosting plan is a Windows based one.
3) All information contained in attached brochure to be included in the site.
4) Main site logo is to be the same as that on the brochure.
5) Use brochure images on site. If these images are too low resolution to use on a quality web site, then I'm open to using other legally sourced appropriate images.
6) Include a Contact Us page that includes the following fields; First Name, Last Name, E-Mail, Comments.
7) Use ASP include files for footer, menus, etc. in order to keep any future editing work to a minimum.
I'd like to see a few similar sites that were completed. I'm also open to suggestions as to how my above suggestions can be improved on.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
