Database Conversion and Formatting

There are hospital cost report databases that I need to convert into a standard format. These databases are publicly available in the United States. This is a link to download the databases by fiscal year. I am only interested in the HOSPITAL databases for the last 5 years (2010 – 2014). Please focus on the “Hospital-2010” format. [login to view URL] I would strongly advise that you download the 2014, Hospital-2010 database, because it is just for a partial year and does not have that much data. The structure will be the same and it will give you an opportunity to see if you can manipulate this data. The data in this database represent values in a cost report that every U.S. hospital files that looks like this: [login to view URL] As you will see in this PDF, the cost report is divided into different worksheets (e.g., Worksheet S-2), and each worksheet has columns and rows. There is ample documentation on the government website for these databases: [login to view URL] I have a list of specific data elements that I would like extracted for each year to put into a comma delimited database with the following fields: 1. Measure Id 2. Provider Number 3. Data Year 4. Value This is the old measure list (Initial Measures) that I have, that have Measure Id and some direction as to where to find the data elements in the cost report, as defined by the “CR Source” field. [login to view URL] Each field needs to be verified as the location may have slightly changed. Once changed or updated, this table should be updated to reflect the current mapping with the Hospital-2010 reform form. If the location is not correct, the Measure Name should be the same and cost report worksheet is probably the same. There are 2 other data field files that I would like captured in our master database that has the 4 fields from above. Each of these files may contain data elements that are ALREADY mapped in the first measure list file. If this is so, you can ignore the field on the secondary measure lists because they are already captured on the initial list. If the field is a new field, then assign it a new measure number and include it in the master measure list. Besides creating the databases listed above, I also would like you to provide all documentation and create a program so I can download and convert these databases as they are updated in coming years.
