Constituent Management

I need an online application that will be used to maintain a contact list of all the voters in my constituency. I need to keep track of everything that is important with these voters eg. Political affiliation, important concerns, interactions(meetings/visits), issues raised (keep track of these to ensure they are resolved), ability to upload documents that are linked to the constituent, ability to import constituent data from the official voters list for the constituency, ability to manage multiple constituencies. All data must be stored in a relational database. A simple set of reports will be required as well e.g. a report of all issues raised and their current status, due date etc or a report of all voters sorted by street name, or a report of all voters grouped by political affiliation. If the reports could be generated by a simple report writer that would be better.
