Create A Landing Page + Iframe Form

I need a landing page created. The page should have attractive graphics so you'll need to send some samples of your work so I can be sure your design skill meets the requirements. The page is relatively simple and only requires design for a header, footer and some images. I will also provide a link to a form that will have to be framed on the page, as well as a link to a similar site so you can see what it should look like. The framed form is a must, so if your bid is for design only, please specify that up front. I'm on a short time frame for this so the finished page will need to be delivered within 48 hours of project acceptance. Budget is $100 or less. I'll send the examples via PM so you can get a better idea of what the finished page should look like. Thanks
