Custom WordPress theme and logo for my personal blog

I would like to have a custom design for my personal WordPress based blog. I have specific design guidelines for the design, but no advanced requirements for the WordPress theme itself. I would therefore expect that it should not cost too much for applying the design to work with WordPress. I expect a 'shiny', vibrant design so you need some good Photoshop skills to make the design. Supply me with portfolio so I can see that you have done this kind of work before. I will give the following input to the design: - Colours to use. - Width of columns. - Prototype with ForeUI. I would like to be allowed to comment on the design after I get it and then my comments get applied, but only once. I will not be able to make additional comments after that. Because of these terms, I think you don't have to calculate much risk into the price, I do have high expectations of the quality, but won't be a difficult customer. As part of the theme, I would like a simple one or two-colour logo for the blog as part of the title, of a man thinking (with a fist under his chin), while looking at a computer screen - on the computer screen should be two boxes and an arrow from one box to the other.
