Build a email form sending website

Note: This is a barebones description. Much more detail can and will be provided to chosen Freelancer! Thank! Page 1: Login page Username / password login page with usernames and passwords populated from database. Page 2: Main page Bar at top right (listing name, username, link to settings page, and logout button) Below that: list of Sent forms and button to ask user if they want to generate a new one. Page 3: Branch off of Page 2 (New form) New form page: to include boxes to write in information. Need fields for full name, phone, address, Id number, referring user, notes. Next on the page will be a search function to send the form to. Recipients can either be found via search of users already in database or added manually. If the recipient is not already in the system, their email address can be added manually by the user. At the bottom of the page will be a box with TBD text and radio buttons (agree / do not agree) and send / cancel buttons to sent the form or return to Page 2. Email Form: The email sent will be from the user that logged in with the Full Name from above as the subject matter, the TBD text included in the message plus any added notes from Page 3. Each individual user that a form is sent to will be be blind carbon copied (BCC'ed) on the original email -- the sending User will be in the "To" field -- so they are sending it to themselves so they have a record. Page 2+: After form submission After a user has sent / submitted a form. A form is then added to their list of sent forms, and they are returned to Page 2. Settings page: Allow user to update conact information (Name, email address, phone number, etc.). Also allow user to change password after entering current password. Need authentication email sent when password is forgotten -- to email address on file. Admin page: Add user, remove user, reset user password (never see passwords though). Email admin any time a user is added.
