Well, a SIP server can be hacked two ways - one way is to look at it as a plain Linux server that gets brute forced or more rarely a backdoor, and the other, most common type of "hack" is actually some kid of abuse over the SIP accounts (ends up with unauthorized use of extensions or trunks, that can result in high bills toward's the peers).
