I need a application/registration form done in contact form [login to view URL] are questions with multiple answers and some drop downs required. I must be able to edit the dropdowns.I will supply images of the questions. When live My website header and footer must not be visible as the website is still in development.I will supply an image to use as the header. (no links) I will require the coding when done.( This is reposted due to a messer.
