Build a webpage with a test on Shopify website

I need to add a test to my Shopify store.
The test would be similar to these urls provided underneath.

Link 1 test is comparatively simpler with only 4 sections- 3 tests and result.
Also, at the end of all tests the results should be e-mailed to the user. Hence a popup or page with lead capture needs to be presented.
