Please make modifications to my donation form

I've uploaded 3 screenshots of a simple interactive donation form I created myself ([login to view URL], [login to view URL], [login to view URL]). I will provide the actual working source code for the above screenshots to the chosen freelancer(s) - which consist of 2 files, [login to view URL] and framework7.ios.css. The files I will provide are functional - you can click on the "Next" button on the first (Choose Amount) page and it will go to the 2nd page. Clicking on the "Next" button on the 2nd (Information) page will advance to the 3rd page. The [login to view URL] file I've provided uses framework7 and bootstrap for the seamless page-to-page functionality. I'd like the following minor modifications done to my files (screenshots above): 1. Please add "Raised", "Donations", and "Goal" headings above the top bar (with live numerical values above the headings) that will update in real time for all 3 pages each time the donation form is filled out and submitted. 2. On the 1st (Choose amount) page, make the donation amount boxes clickable/selectable. So I can choose an amount I want to donate. Please also add code so that clicking on the "Write a personal message" will drop down a hidden text area field where I can write 3-4 lines for a personal message. Clicking again on the checkbox will hide the text area field. 3. On the 3rd (Payment) page, clicking on the "Donate Now" button should submit the form fields for all 3 pages to a mysql db table. So you will provide a simple php file and sample mysql db table for this back-end functionality. 4. After clicking on "Donate Now", the "Raised" and "Donations" figures should update in real time above the top bar for all 3 pages. 5. Add a form color picker table to the [login to view URL] page (link it to a button and place it to the right outside of the form) so that I can choose any form color and the current pink form color will update in real time to the chosen color. Please do NOT place an instant low-ball bid just to start a negotiating session. Whatever you bid should be the final price you are willing to do this project for. We are looking for a long-term relationship with the right coder and have several projects in the pipeline.
