This spreadsheet is for a self assessment by users. Rules and conditional formatting will produce automated scores and matrix chart. See attached spreadsheet. REQUIREMENTS: 1. Column H. The cells in Column H, must have a red/amber or green colour for each row item, based on the rules given in the sample matrix 2. Executive summary 5 x 5 matrix. Each row item short code needs to be plotted as shown in the sample 3. Executive summary total score calculation with red/amber or green colour coding. Risk Score as % of Total Possible Score (TPS) needs to be calculated, and then coloured as per the following rules: =/> 50% of TPS: RED (Top Priority, needs urgent attention), Between 25% and <50% of TPS: YELLOW, (Medium Priority, needs selective attention) <25% of TPS: GREEN: (No systemic risks identified) 4. OUTPUT: the speadsheet must not be editable, formulae need to be invisible, and users can only enter values to view the resulting scores, and print out.
