Create eBook from PDF

I have a printed book and the PDF and ages ago I also had an eBook with Mobi.
I tried several times to convert for Amazon and/or Kindle but couldn't persevere long enough.
The print version is listed at [url removed, login to view] and is available at many libraries in Oz.
Please quote to convert the PDF to a Kindle (and whatever).
Also, do you look after getting it onto Kindle eBook site (and others)?

Do you ever quote being paid with royalties? Maybe 50% of sales? Just supposing:-).
