Online survey design

Hello, we want to switch from a printed (hardcopy) process of evaluating something to an electronic form. Background: A group of 10-20 people form a jury. They evaluate a demo they see by a survey / review form by answering around 10 Questions (5 sheets in printed version) . So I taught to create a google form or whatever to change from the printed version to an electronic version of the survey. The form itsself is pretty simple, but need the following features. 1. The form need to be filled by a closed group of predefined 10-15 different perople and it must be clear after receiving the electronic answers whch form was filled by whom. Is that possible with google forms? 2. How are the users (jury members) "invited" - they get a link to the google form by email? Does every user get a different link or are they using all the same link. 3. does each user need to have a google account? (user means the guy who is filling the form) 4. do google form provide a summary of the entries?
