Modifications to simple Flash contact form

I am attaching 2 screenshots, 1 of the current form and 1 of the modifications I need which I describe here: 1. I need to add a tick box where the visitor will tick to confirm he understands the terms 2. A text box with the terms with a scroller as in image number 2 3. The 'Send' button should be locked until people tick the box, then it will become clickable. 4. Before the send button there will also be a _blank link to a HTML page 5. The rest of the code should be left untouched for the form to work as it is now. Let me know if you have questions. Thanks!
