Build a Clearance Process Website for Proof of Concept

I need a website to be designed and developed in a very simple and straight forward way. I need it fully functional based on the attached requirements and I will like it done rather quickly. I will certainly give a bonus payment if i can get it all done within a maximum period of 5 days. I will be fully available to answer any questions. The process works as follows:
- student registers on site and email get validated
- student logs in and is presented with 12 pre defined forms to complete and submit each form
- admin receives the submitted forms and approves or rejects; student will then be able to see decision.
- process is closed.
It is quite simple and further details are in the attached. I was disappointed by another freelancer so hoping this works out better this time around
