Acupuncturist Directory for the island of Oahu in Hawaii

I desire to build out my web site [login to view URL] - the finished version will be like a modified version of [login to view URL] but it only focuses on the honolulu area. I would like this project to be done in classic ASP and Access database so that I can make minor changes myself since I am familiar with these technologies. All forms a pages need to appear highly professional and again please stick with classic ASP. I have already created the Access database, and you can visit the existing home web page to see that I have already created it (less work for you!) Here is a summary of the work I would need done within the scope of this project: a) a 'find an acupuncturist' page that lists all of the different search methods in summary -> search by location, search by education or specialty, search by insurance accepted, search by house call availability, search by price, search by researching acupuncture treatments b) a contact us page that includes a simple form that emails me their name, phone, best time to call, email, and message content c) a page that shows a single acupuncturists complete profile, map location on google maps, and links to their specialties information d) a page where acupuncturists can "claim their listing" by verifying their email address and entering a secret code into our web site that is emailed to them e) a page where acupuncturists that are not listed can add themselves to the directory as a new listing - also requires email verification. The listing is 2 parts first part is f) a page where acupuncturists can use paypal to buy a one year membership that will allow them to become the FEATURED practitioner for house call. This is a simple form page No need to do a paypal API just a simple paypal payment link to my email, then I will contact them via email to activate their payment it. g) a page where visitors can browse acupuncturist by their location (neighborhood) - this page will use google maps API to show all of the acupuncturists in that neighborhood on a google map. h) a page that returns the results of the access database to list practitioners according to their education types, degree types, and specialties i) a page that returns the results of the access database to list practitioners according to their insurance types accepted J) a page that returns the results of all acupuncture practitioners that do house calls K) a page that returns a listing of all acupuncturists sorted by price (low to high or high to low) L) a page that allows users to research acupuncture treatments by displaying a list of all of the conditions that acupuncture treats. THese are listed in the database as M) a page that shows a single treatment condition such as "dizziness" - includes a short description of the disorder, and how acupuncture can help. At the top of the page is the 'featured practitioner' with their photo, followed by a list of all other practitioners that treat that condition. N) I also want to change one of the icons on the home page - please change the search by price icon that looks like a bomb to look like a dollar sign instead : "$" O) There may be a few things I've neglected to mention but you should get the idea by now. I included a copy of the database so you can see basically what I mean, and also a map of the neighborhoods in Oahu (the search by location will be sorted by neighborhood before redirecting to a google API that displays all of the addresses in that area on google maps.
