Style a Wordpress plugin

I require someone with a good understanding of user interface best practices, as well as a creative approach to website design. If your profile doesn't have a profile of your work, share a link for me to view some previous work. I will split the project into multiple milestones, the first milestone is actually separate to the plugin, but for the same website. The client has a 'Contact Us' page with is rather plain looking and was not made to be responsive, so just didn't work on mobiles. I made some minor improvements to get it responsive, however I want to give this page a more modern feel. It is a wordpress page, and there is a relatively untouched Jollyany - Corporate Multi Purpose Wordpress Theme in use, I will add required CSS to the child theme for this milestone. In the attached image '[login to view URL]' you can see the basic layout as it currently stands, there is not a lot of content, however I would like each of the people to appear as an individual contact card, replace the words email, phone etc with symbols and maybe have each looking like a business card with some colour (theme has a highlight of #ec5540). You can always impress me with a mock-up if you wish. / End first milestone Additional milestones will be agreed on for the following items, which relate to the styling of the plugin. This will require both HTML formatting and CSS which I will later implement into the plugin myself. I have a private github project which can be used to collaborate on this. There are several pages created with shortcodes in this plugin, and much of the contact is dymanic.. however there are 3 key elements I wish to be styled and each must be responsive. Milestone 2. Format and style an input form, containing text boxes, drop down menus, checkboxes etc. I want this to be extremely user friendly as there are quiet alot of text fields and will sometimes be used via mobile devices including iPhones. Current html markup is bare with just <label></label><input><BR/> for each line. Need advice on splitting the form sections into pages (hide/show parts of form) with next/back buttons to ensure user friendliness. Note. the form uses HTML5 for validation. Milestone 3. Format and style a HTML <table> ensuring user readability on all screen sizes, again advice on approach would be needed. Then further use the same styling on a javascript 'Datatables' table, consistent with prior table, however has addition of needing some extra elements such as search, sort orders etc. I have attached '[login to view URL]' which shows the datatables table as it is currently formatted (only by the theme styling) I will likely have some additional milestones which can be negotiated down the track, including styling of a custom menu.
