Create a Setup file for Mac OS X x86_64 (Apple)

I have a jar file for my application and i want to create a setup file(.dmg/.pkg) for this jar.
The installation should show below 2 screens:
1. Licence Agreement screen which will show the licence text for application
2. This screen should provide a facility to the user that user can change shortcut icon name.
You should provide the details how you generating set up file, so that i can create set up file in my development machine when there are changes in my jar file.
