Dynamic Questionnaire Form with bar chart results

This project is about developping a Financial Health diagnosis tool for businesses. This tools asks around 50 questions to the business owner, and based on his answers, automatically delivers a scoring for several fields of his company. This project is in french, but it shouldn't be an issue, essential elements of the project description have been translated to english for clear understanding, and you may translate other elements as the questions for instance with google translate to have a general idea of the content of questions. Basic features : Here is how it works : 1. The questions are displayed 1 by 1 within a user friendly and dynamic form, integrated in a wordpress page (page already created). 2. When all questions are answered : -> it calculates automatically the scoring (each answer adds points that lead to a scoring on several key areas of a company : Finances, marketing...) -> it shows the result as a bar chart on another wordpress page (also already created), or on a page outside wordpress if technically required -> automatically sends the results in a simple email (detail of all results question by question + global scoring) to an email address dedicated to it. Optionnal features (not sure of feasability - make your bid price without these options, and mention the full bid price with these options in your message) : -> automatically sends this bar chart with some text in a pdf attached to the email of the business owner (email may be "catched" if we manage to pass in the url, as it is entered on the previous page). -> automatically adds the email of the business owner to an email list on the so called "SG Autorepondeur" emailing service. An easy API exists for it : [login to view URL] Rules / Technology to use : - have a consistent and clean look and feel on all major web browsers - Follow the design template provided for the dynamic form - use a good looking chart library for the bar chart (we will validate if it is at least reasonnably good looking, but there are many nice libraries out there for it). - A first analysis has already been carried out, and obviously javascript and jquery have to be used to carry out this project. Jquery may be especially used to generate the bar chart (and the optionnal pdf file generation). - However, if you feel like using some other technology that you believe is more efficient to realize the project, you may use it. Just keep in mind that this should be integrated at the end on a wordpress website. - A clean and commented code, so that we may make little changes easily if required (we may change an picture or a text in the future for instance). The work on this project has to start september 7th (we can not followup the project before), and has to be finished before september 25th.
