Google Docs integration w/Javascript -- 2

We are looking for someone to integrating a Google Sheet with Google Doc using some great code we have developed. Javascript expertise is a must. Here is the end result we are attempting to accomplish:

1) Google Form will populate a spreadsheet with fields

2) Google Sheet will execute a script

3) Script will populate fields into a Google Doc template file and create a new file with these fields from the existing template

4) Populated Template File will be unique and be created in a designated Google Folder

You will not need to write the script. You will be using the script here:
