Javascript to Insert Data to MSSQL upon web form submission

I'm looking for a javascript code that will collect a session reference (already exists), IP address, and date / time when a user submits a form through our website, and insert that information into a Microsoft SQL database.
The script can use the Maxmind GeoIP Web Service ([login to view URL])
