Looking to create a word document based on a static PDF. The word document has been derived by converting the PDF into a word document using Adobe Acrobat. We would now like to change the structure of the Word document so that all of the fields in the document can be populated. At present the various areas of the document have been created using Word shapes/drawings. We now want to use ActiveX controls in word (checkboxes, textboxes etc.) which can be filled in electronically or later prefilled from a database or excel.
Attached are 4 files:

> an excel spreadsheet where we would ask you to keep track of every field/control added and describe which page or question in the document it corresponds to
It is ESSENTIAL that when printed the word document looks *very* similar to the original PDF when printed. It is ESSENTIAL that you keep track of the names of the objects/fields/controls you add to the word document in the spreadsheet provided. You must have experience with ActiveX controls in Microsoft Word. You must use the ActiveX (not Forms) controls available under "Legacy Tools" in Developer>DesignMode>Legacy Tools in word.
