I need to create a form in outlook 2010 that creates a unique number each time it is activated in a field and this and a number of fields on the form export there data into a spreadsheet on the network. If some of the fields can have pre selectable values I can populate as requirements change also. The email needs to go to external clients so

It is easier and configurable if it is presented as an Outlook Addon. This addon will create a custom form on your outlook. And the fields properties can be configured as well.
