Need AdMob Integrated into an app

This is a very fast job. I need two things done:
1. I need you to use AdMob and put it in the areas I show you.
2. Make a box that will appear after 5 minutes of use asking the user to rate/review the app.
You must connect to my PC while doing this since I don't feel comfortable sending my source code out. Need to get this done pretty fast.
