Open Source Script Customization (PHP Google Store Locater)

We are in the process of creating a website program for our patients to find practitioners in their area Basically when a patient enters a zip code, all of the practitioners within 50 miles of that zip code are listed and displayed on a Google Map. We also have a way for practitioners to add themselves to the map. When the practitioner submits all of the required information (name, location, email, phone) an administrator can approve or deny the listing. Once approved, the listing is displayed publically on the website for patients to see. We are looking for somebody who can help us troubleshoot this problem or recode everything. We are currently using an Open Source PHP Program called PHP Google Store Locater. It can be downloaded from CrystalDawn.net. Here are the requirements: 1. The Google store locater must match the theme at PRRTPatient.com. 2. Patients must be able to search for Practitioners within 50 miles of their zip code. 3. Thousands of practitioners must be able to enter their information on a public form where all the data is stored for the administration to review and approve or deny. 4. Practitioners must be able to enter this information: a. Name b. Clinic Name c. Address d. Website e. Email f. Training Level (Basic, Intermediate, Advanced) g. Specialty (MD, PhD, Physical Therapist, etc..this is already built into the open source script mostly) h. Clinic Hours i. Personal Note j. Picture 5. There must be two public forms. One form will allow practitioners to submit all of their information except a personal note, website and picture. The other form will allow practitioners to submit all of their information. 6. I need to be able to speak to you over the phone about this project. I have a Skype name (brendenwilson) and I would prefer to use that. 7. The program must also verify the address to be sure that it will show up when searched for. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Server Side: Linux, MySQL, PHP Client Side: Must show up in IE, FireFox and Camino.
