Looking for someone to do a small php project for me i need a order form for ordering game servers. it must have the user fill out a form then email that form to me and then send them to paypal so they can pay for their game servers the form must include the following information User information first name, last name, address, city, state/province, postal/zip code, Country Contact information regular email address, paypal email address, Game server Information (drop down selections) Game Platform, Number of players, Type of server, Server location this form also needs to calculate the price or the server and display it as they change server options. i.e private servers are less then public servers. all pricing is based around the number of players depending on if it is a public or private server. after the form is completed and the price is displayed the form must send the user to paypal with the selected pricing for the server they selected. and send me the email with the options they selected on the form.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
