box lunch order form sent to email address

Hi I need to put together a box lunch order form that is very small, but needs to be able to as several questions and pass that info to and email in an nice easy to read/formatted email. Below it the menu for the box lunches. the prices are all the same but their are several choices you can make in several categories. I think one thing I am curious about is how in the beginning to choose how many orders would need to be made. I guess you could choose 6 orders from a drop down and then 6 order forms to fill out and customize would show up for each one. and or have it so you could say in my order i need x box lunches, then have boxes saying like x of one kind of sandwich with x bread, x of another etc.. and then they could do the same for each other item you would choose from. Or perhaps there is a better way and I am just missing it. I just don't have the time to do it now. Sandwiches: Turkey Gobbler Turkey breast, swiss cheese, cream cheese, lettuce, tomato, red oonion and honey mustard. Brenen's Club Turkey breast, ham, roast beef, bacon, Swiss cheese, lettuce, tomato, red onion, sprouts, spicy mustard, and mayo. Californiam Veggie Swiss and muenster cheese, lettuce, tomato, red onion, cucumber, green pepper, mild banana peppers, alfalfa sprouts, ranch dression, and a layer of our special vegetable dressing. Turkey Plus Turkey breast, Muenster cheese, lettuce, tomato, red onion, green peppers, mild banana peppers, alfalfa sprouts, and Russian dressing. Breads Thick-thick Italian Sourdough baguette Mulitbrain baguette Thick- sliced wheat Wraps: Southwest Chicken Sliced chicken breast , black beans and corn salsa, lettuc, tomato and a layer of our special vegetable spread wrapped in an Italian herb flatbread. Chicken Caeser Sliced chicken breast, bacon. Swiss cheese, letuce, tomato, red onion, and our house Caesar dressing wrapped in a Italian herb flatbread. Greek Veggie Crumbled Feta cheese, cucumber, tomato, red onion, breen peppers, mild banana peppers, black olives and Greek dressing wrapped in an Italion herb flatbread. Chicken Bacon Ranch Sliced chichen breast, bacon, cheddar cheese, lettuce, tomato and red onion wrapped in an Italian herb flatbread. **Any sandwich ma be served as a wrap.
