Registration form overhaul

Hello GAF Bidders - Client wants to automate the signup process with forms for her afterschool daycare for children. traditionally they have been downloading everything by hand .pdf and is gettting to be much to manage. They have a new employee who seems to want to do it according what is below. I think she uses "filemaker" Anyways, please look at this if interested and give me your feedback on this method and/or maybe a better one. The following should be the information you need to get you started.(please see attached files for more info) I am sending two emails, the first is all the requirements and database information. The second is the blank forms in either word, PDF or ASP. I tried to be as specific as possible. The two tables are P and C. I have numbered each field and placed them on the forms. I only use half the information on the forms for my database but will need the user to enter in the information for the print out. The print outs formats should look the same as the forms once printed out, as well as the email output. Our registration for the following school year occurs in late winter/early spring.
