I need a simple form created to submit 2 values to a MySQL Database, the values will need time/date stamp also. 1st form field is a number to be selected from a drop-down list. 2nd form field is to be an email address, must be unique, will need to check the DB to ensure unique. If not, error. Page must use AJAX or CSS so the form is submitted and page is not refreshed. Results will need to be displayed on the screen after Submit. Simple captcha will be required also. Please see information attached. Design is not important, form functionality is.

i previously did ajax forms like the one you need. i am the best one to make ajax forms. the form will be submitted using ajax and the form will tell the user that he succeeded or not if he put wrong email or worng captcha code then the captcha code will be refreshed also without the page to refresh.
