554927 Web design for form

Hello, I am looking for a web designer who can modify an existing "selection form" to make it look prettier. The form is located here: [login to view URL] Please note that the form is NOT working, I am aware of this. I do not want you to fix the form itself or change the text, I only want you to change the web design. If you view the source of the page you'll notice the current HTML coding is a HUGE mess. I will personally implement your modified work into the live website, where the form is working correctly. I would like you to keep the design a bit similar, however you will notice that the colors do not fit well, the text isn't aligned correctly at all, the button is too close to the side etc. etc. I would like you to change those to make it look more clean. You may change the CSS, jQuery or images or anything else as you fit, as long as it will look better. The current CSS may not even work well on other browsers as it hasn't been tested.
