Webhosting Order Management

Hello, We need to process web order form which will be processed in 4 steps. Following are the steps: Step 1: Credit Card Number (without spaces) and Email address ---> user press submit button, his credit card is verified by online merchant or is checked by algorithms. I prefer online merchant system. if card passes the validation process user will be transfered to the next page. Step 2: Full Name on Credit Card , Credit Card Expiration Date, Credit Card Verification Number, Credit Card Bank Issuer ---> In one table cell which will be placed on top it will show the last 4 digits of credit card which was previously entered! like if user entered 1234517895893699 it will show xxxx-xxxx-xxxx-3699. Step 3: Street Address, City, State, Zip, Day Time Phone, Country. ---> In one table cell which will be placed on top it will show the last 4 digits of credit card as well as bank issuer..State in combo box will be shown only of the country selected. Like in country user selects Canada then the page will only show canadian states. Another thing on the same page is to show the Flag of country which is selected!.Like if the user selects United Kingdom in country it will show the United Kingdom flag on the page. We can provide you all countries [login to view URL] if user writes the telephone like this 456789123 it will be converted automatically to this form using javascript (456)-789-123. Step 4: Hosting Plan, Contract Length, Desired Password, ReEnter Password, Secret Question, Secret Answer, Promotion Code, Agree to terms check box. ---> In one table cell which will be placed on top it will show the Billing details which user already [login to view URL] pressing "Process Transaction" button it will take the user to the Thank You page without charging the credit card as card will be charged manually after verifying the customer via phone. Automated Phone call system will be added to the script by our self to verify the customer. Required Things On The Script: - Should be in PHP, HTML, Java Scripting, Java Applet to get real IP (if needed) - All fields in the whole script will be compulsary! User cant go to next page without leaving any field blank! - Errors will be shown in RED colour near the mistake in the field and will highlight that field too! - If user closes the window of the order form it will automatically popup a new small window where he/she will be asked "Please Note your Hosting Order will be cancelled.....etc etc" And on the poped up page he/she will see Live Support image, phone numbers, or special offers so users get more attracted and click on the link to open the hosting order form again. - After user enters his billing details and he want to change something and clicks the back button the script must tell him the previous information he entered for his billing address by re-entering his form automatically. - After user fills out the form it must email all details of the customer to the sales department including his real IP and country based on his IP. - Must be *Template Based* . We can change its design, css, header, footer, body all things anytime with ease. - Email which will be sent to sales department should be template based, as well as the invoice sent to the customer should be template based too! like we just edit [login to view URL] and the script will use that file to send invoice messages to the customer. - Code must be completely commented! - Admin should be able to block fraudulant customers IP, so they are not able to use the Order Form. Custom page will be there to show the HTML text to the Banned IP(s).
