QGIS Map Viewer / Visor Request

Need a visor interface to request maps with QGIS website interface for Linux.

* Visitor need to sign up or login to request a map report

* Visitor must see google map and write some basic information like:

email (from existing user to send PDF report), radio lm (1,2,3 km), address, neighborhood, city, state or choose a map point, next to a google map window to confirm place.

* Confirm map address and radio

* QGIS must generate a map view for radio choosed and labels for 5 (or more) existing shape files, one map and labels for each sheet.

* Send PDF by mail anda data confirmation (address, neighborhood, city, state, radio, coordinates)
