5. 1 form design in CSS
6. HTML form & Javascript provided but the Javascript will need to be updated to pull data into our CRM system
7. The form needs to be able to pull data from the source URL in order to track where visitors are coming from
8. This data needs to info upload to our CRM system and inform where the user should be redirected to
9. The templates need to have the same responsive design as the existing template pages and the same left/right margins
