Build Sliding Multi Step Form For My Website

I need a web developer to integrate a multi-step form (wizard) with validation, and postcode lookup functionality (using Ideal Postcodes API) in to my bootstrap responsive website.
Three step wizard form:
First section has three select boxes.
Second has Title (select box), First Name (text field), Last Name (text field), email (text field), phone number (text).
Third has postcode lookup box, then select box appears with address selection.
The form data will need to collate in an xml file, and re-direct user to a thank-you page.
A relatively straight forward job for someone who knows there way around Javascript
with experience.
