Build a Website with Database

Hi, I need to build a website with database for daily inspection. Here is what this website can do: 1) I have a lot of products, they can set as Lot Number, for example Lot 1, Lot 2 2) My business is the arrange client to come inspect my Lot, and I want to record what they have inspected each day and how many times 3) So for example, when a client walks in, I will take his name and the Lot he wants to check (input field for name, check box for Lot) 4) Once he finished the inspection, I can hit submit and store the data 5) The same client might come again on the same day, so I will input his name and checked the Lot he want to inspect (here I want the input field able to search the name I have previously submit, so for example client SAM come again, when i type in "client SA" it should able to let me choose "client SAM") (and for the same Lot he inspected, when I hit submit, in the previous same Lot he inspected will increment as "2" where he inspected 2 times) 6) The inspection can go for weeks, so for example Day 1 client SAM came once, Day 2 client SAM come again, I will able to know 7) Lastly, just need to output csv or any sort, best is do a dynamic table as well so I can sort by Day or by Lot to see my data. NO design is required, just simple black and white with inspection page and goes by date, and another page for output.
