I'm looking for someone to create a contact form for my new website. The form is to capture user data for people wanting to sell their watch.

I need similar functionality to the old site (but would provide a designed layout) which would be in two columns.
I need this page created fairly quickly, it also needs to be responsive, safe and spam free, and follow good SEO practice.
