Create interactive form with payment plugin

You may utilize any popular API like google form, jotform e.t.c. and provide the implementation to me. 2. Form has to be completely responsive in mobile. 3. Form must connect to payment via paypal once it is submitted and the form needs to be delivered via email to US only after successful payment. We do not want form without payment step completed. An email also needs to go out to the person submitting the form confirming his payment and form submission. 4. All results/submissions must be saved into an excel or spreadsheet for tracking 5. Please read below for the fields information Title: Existing Student Registration and Fee Payment (Mandatory Field) Student First Name (Mandatory Field) Student Last Name (Mandatory Field) Parents Full Name (Mandatory Field) Email Address (Mandatory Field) Class going into (Perhaps have an info thing here..where when you point to it, it reads "Select the new class that the child will be going into this year” . For those going into class 7, please select class 6. There is no class 7 this year.) Fees: We should show a breakdown here: Tuition: $120 Security Fee : $20 _______________ Total: $140 (This is a Mandatory field, and should be selected already. Everyone filling this form, has to pay the $140. No option to unselect.) Text Books: (These are optional fields - each book should perhaps have a checkbox..and based on how many were selected it should total up. See enclosed file, for a list of books for each class) Please Note: Below are text books required for the new class that your child is going into. All books shown below are mandatory. However, they may be instances where a book that is going to be used in this new class, may also have been purchased and used in a previous class. If so, you may use the same book. We’ve tried to highlight such books below.
