Summernote upload manager bootstrap-style

I want a very simple upload manager in one PHP script, that I can call from Summernote, a html-editor. As you can see from the links below, it is well documented how to plug this into the html-editor using a few lines of code (I use several editors on same page dynamically with name like summer1, summer 2 etc, so you must of course support this in your provided solution). I want you to implement the button (as documented below) for this call to a .php page where one can choose to upload or quick browse between already uploaded pictures. Also, you should code this: One should be able to crop or rezise the uploaded picture in this quick upload-page. This page doesn't need any special styling as long as it uses bootstrap style and a panel like in my current application. One shall also see 3 images in a row in this pop-up page where you can simply choose a link to insert the image url back into Summernote. Since I need basic crop/resize functionality, you also have to either code or find/test possible already made scripts for this - it should be quick and simple - just drag a square to the position of the image or choose to resize the image to a pre-defined size. This script must be allowed used in commercial projects, so you can't just choose anything. There are already made software for this upload/manager-functionality (for instance elFinder), but I want just what I have mentioned, nothing else and I don't want lots of files and additional folders. This also goes for any resize script you find. Also, I want it bootstrap-style so that I can easy style it myself and I don't want lots of other functionality.
