Build a Web Form

I am in possession of 2 elements. 1) an excel file that does the calculations I need 2) a multilingual PHP webform that already does most of the calculation I want to transform this webform to be able to do the same calculations than the Excel sheets. so basically: 1) understand calculations on the excel (Leasing calculations with one specificity tied to the first monthly rate) (the two first sheets are identical, the only thing changing is the language. The third sheet is a reference sheet, should be integrated in the form, as are the Residual values) 2) delete all client input from the webform (except maybe email to be used in 4)) 3) add needed calculation steps and possible services as options with according price to reproduce the calculations done in the excel file. The Calculation steps include getting a residual value from a table depending on duration + KM, calculating a leasing rate, and adding fixed price services to that rate. A little cosmetic to make it look nice would be perfect. 4) Option to print out the form as it is seen on the screen (somewhat like the excel) and to send via email in a nice responsive email.
