Create a Basic MySQL Database & PHP/AJAX (Design Completed already)

We're looking for someone to code up a basic SQL database which we will use to check items in and out of our inventory. We've already "built" the system on a basic AJAX template, but it's not functional. We are looking for someone to code in the PHP required and build the SQL DB/layout. The system consists of 3 pages: - Login (Users type in their username and password to login) - Check in/Out Items (Users insert details about the item) - Item Status (A running list of items checked in or out of the system) The user will login to the system with a username and password (we don't mind creating users in the database as we won't be adding users often). The users table will need to consist of: - Username (2 character max) - Password, ideally MD5 encrypted, but not too fussy as long as there is some sort of encryption Once logged in, users will be taken to the item status page. This will contain all the items contained in the screenshot. This one is pretty self explanatory as to what it needs on it, but it's pretty much a log of devices / equipment coming in and out of our location. All of the details displayed on this page would be generated on the other page (Check in/Out Items) except for the date, time and username which would need to be added in based on the session and server time when users Check in/Out Items. On the Check in/Out Items page, the user will enter the tag number. Ideally, if the tag has been entered in the system before, the page will populate the category field and the info field for the device to avoid us retyping it, BUT if the user chooses to type over the category or info, it'll just save the updated parts into the DB. Once the user fills out all fields and clicks the button, it'll add it to the DB and appear on the Item Status Page. We may need to add categories and Statuses from time to time, but we can do this from PHPMyAdmin or similar to make it easier. I've uploaded screenshots of the 3 pages.
