create a forget password functionality using jQuery and PHP

Okay i have a small db of users i want some one to use jQuery/ajax to create a forget password html page with php scripts to a db. you can add additional things to the db to make it suit you approach to the idea see my db attachment. Note don't change any field within the table but u can add i will need this in two days.
Description:
user must enter their email where this is confirm through ajax by posting the email to the php server and the server must send an email to their email provided thats all.
Note the html file an the php must be separate file as i said it must use ajax/jQuery.
