Duplicate metabox dynamically

I am in need of someone to create two similar metabox ; one for home team and another for away team. First element will be for player name; from custom post type football_palyer Second element will be event type as: Third element will be for minute All three elements will be clone able ; if I click on the add button then these three elements will be cloned ,and if I click on the remove button then all three elements will be removed. For details see the image. My budget is $10 for this job.
