Magento 1.9 add multiple products to cart with notes from custom page

In a custom form on magento 1.9x page we have 3 select boxes with products. Each option has name, price and id of product.
User can choose any of that and than another select box with quantity.
Eg for qty = 5 we will have 5 x each 3 select boxes/products.
Next we have 4 checkboxes. x,y,z and other (please leave a note)
If z,y,x is checked than I need to add this text to main product in cart.
Same for other, that is a textarea (custom text written by user)

This can be done with querystring I think - site/checkout/cart/add?product=id&qty=1&related_product=id2,id3&related_qty=qty2,qty3 and notes.
