Online event registration programming

I currently have an online race registration database for my running events integrated into my event websites. I am adding some events that are different so I need to have a a new database built. My current events involve teams of 6 and 12; my new events are simpler and involve teams of two. There are currently three different events and I plan to add more, so it must be easy to add additional events. For each of the participants, I will need: First Name, Last Name, Address, City, State, Zip Code, Telephone Number, Email Address, T-shirt Size (drop down box), Age (drop down box), 5k time (drop down box), and how they heard about the race (drop down box). The team will also need to select their category. Following all this, there will be the text of the waiver so that they can sign it and mail it in (see attached hard copy). Thus, there will need to be a button to "CONFIRM REGISTRATION & PRINT" so they can print out their registration to mail it in with payment. In addition, a page will need to be built that lists all the registered participants online. I also need an Admin only access page to the database so I can make any changes, and a REPORTS PAGE set up so I can view online AND download various reports into a spread sheet such as: all data, email addresses, t-shirt sizes, total number of t-shirts for each size, how they heard about the event. It would also be nice to have a check box feature that I will check once payment has been received - once this box is checked, then their information will shop up on the Registered Participants online [url removed, login to view] will need to be a drop down box so I can easily switch between the events. See attachment for my current reports page. I will provide the html template that the coding needs to be done on, or it can be done so I can copy and paste it into my html pages. I am looking for this to be completed in a timely manner. Prior experience online registration databases is a plus, so please mention or send me examples. If the right person is found before the job posting is over, I'll pick them so we can get started right away. Having a SKYPE account with voice so we can communicate easily during the process is preferred. There may be other features I think of as we go along, but I am fair and will compensate the programmer for any additional work not mentioned or is not included in your bid (see my ratings).
