Wordpress pluggin needed

I need a wordpress pluggin that I can install on one or more websites at will. Basically, I need a form page for a Non-Conformance Report to be filled out by authorized site users only. Most users will not be allowed access and I need to be able to set this as a site user role at will. The form will always look the same but, when completed would be sent to various other users that would then responde to the NCR with thier plan to make the appropriate corrections. Then the form would be sent back to the originator and the Quality Department for review and approval of the correction plan. If rejected the form would bounce back to the person who wrote the correction plan for revision. If approved a notification would be sent to the person who wrote the plan that it was approved and that they should implement the plan within a certain number of days. The quality Department would then schedule surveillance audits to verify inplementation and effectivenes of the plan. When the scheduled surveillances have been completed, the NCR coiuld then be closed. Obviously, all of this is rather vaguely described! I will provide a written specification to whoever i hire and give detailed instrutions and expectatioins. I'm not in a huge hurry to have this completed so no real pressure on time limits. Quality and functionality are MUCH more critical. Also, you must have a willingness to go back and fix any bugs discovered after the plugin is delivered and paid for.
